Nurse Practitioner at MJHS
Job SummaryJoin MJHS as a Nurse Practitioner leading primary care services for Advantage members in long‑term care facilities and at home.
LocationRochester, NY
Base Pay$140,000 – $150,000 per year
Compensation Benefits$15,000 Sign‑on Bonus or Student Loan Assistance!
Key Responsibilities- Collaborate with physicians to provide and coordinate primary care services to an assigned panel of Advantage members in participating long‑term care facilities.
- Develop care plans and ensure access to preventive care services in line with member and Plan health outcome goals and objectives.
- Act as a clinical resource to participating providers and facilities.
- Serve as a team member and clinical resource to providers/facilities.
- Contact and support member/designated representatives regarding all aspects of individualized care plans and disease processes.
- Master’s Degree in Nursing, graduate of an accredited Nurse Practitioner program required.
- One to two years of clinical experience in the role of nurse practitioner required.
- Minimum of one year in a geriatric specialty.
- Previous clinical RN experience in acute, sub‑acute, and/or long‑term care settings preferred.
- Prior experience in managed care a plus.
- Valid NYS RN/NP license, registration, DEA certificates.
