City Way Surgery is a friendly, forward-thinking GP practice looking to recruit an enthusiastic and motivated Practice Nurse to join our supportive clinical team.
The RoleYou will play a key role in delivering high-quality, patient-centred care, working closely with GPs, HCAs, and the wider primary care team. This is a varied and rewarding position suited to a nurse who enjoys autonomy as well as teamwork.
Main duties of the jobKey Responsibilities
- Chronic disease management (e.g. asthma, COPD, diabetes, hypertension)
- Cervical screening and women's health
- Immunisations and vaccinations
- Wound care and dressings
- Health promotion and patient education
- Infection prevention and control
- Supporting QOF and enhanced services
- Registered Nurse (NMC)
- Experience working in primary care or general practice (preferred)
- Excellent communication and interpersonal skills
- A proactive, compassionate approach to patient care

Management of chronic disease
- Identifying significant abnormalities
- Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Therapeutic monitoring
- Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach
- Recognising abnormalities
- Identifying the impact of treatment and implementing or recommending changes as appropriate.
Wound Care and management
- Assessing routine wounds including trauma
- Assessing [and prescribing] to support good wound management in complex situations (e.g. use of Doppler technique)
- Identifying significant abnormalities
- Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
- Working with patients to develop a management plan where health problems or potential health problems are identified
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)
Health screening
- Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)
- Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
- Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health
- Administering injections and providing guidance in accordance with guidelines
- Identifying problems
- Provision of specialist and evidence-based support to the team.
Immunisation (adult and child)
- Working with patients with more complex immunisation issues.
- Providing Contraception services
- Providing a nursing care and performing reviews of patients with Learning Disability

Person Specification Qualifications- NMC Registered Nurse
- experience in general practice and dealing with long term conditions
- experience in working for NHS
- experience in working in a GP practice
