Licensed Practical Nurse (LPN)

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our multidisciplinary care team focused on adult and geriatric patients. This position is primarily mobile, requiring travel to patient homes, offices, and hospitals as needed.

Type of Position: Full Time

Weekly Hours: 40

Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.

• Support the Transitions of Care program aimed at reducing 30-day readmission rates.
• Assist with implementing care plans under the direction of the RN Care Manager.
• Perform tasks including appointment scheduling, biometric data monitoring, and patient education reinforcement.
• Attend patient visits and complete home visits as necessary.
• Connect patients with community resources and maintain frequent patient contact.
• Document all interactions and updates accurately in the electronic health record (EHR).
• Participate in care team rounds to provide updates on patients.
• Manage a caseload of approximately 25–30 patients on a monthly rotation.

• Minimum of 1 year of experience in a home health or mobile case management setting.
• Current Basic Life Support (BLS) certification is required.
• Knowledge or experience with conditions such as Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and managing multiple complex comorbidities is preferred.
• Strong communication and documentation skills are essential.
• Ability to work independently and demonstrate flexibility throughout the workday.
• Access to reliable transportation for travel between appointments.

• Knowledge of community resources available for patient support is beneficial.
• Experience in care coordination and understanding patient barriers to care is a plus.
• This position does not involve hands-on nursing care but focuses on care coordination and patient engagement.
