St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver 1:1 holistic in-home care. You’ll partner with an interdisciplinary team to build personalized plans and guide end-of-life conversations with dignity and empathy.
You’ll perform comprehensive assessments, educate patients and caregivers, administer treatments, and participate in on-call rotations. A valid driver’s license and reliable transportation are required as care travels to patients’ homes.
