LPN – Case Manager

Full Time, Monday–Friday, 8:00 a.m.–4:30 p.m.

Hours:

40 Hours per week;
Mobile position requiring travel for home visits, hospital visits, and patient office visits.

Active LPN License

1 year of experience in Home Health or Mobile Case Management

BLS Certification Required

Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities preferred

Support the Transitions of Care program to reduce 30-day readmission rates under the direction of an RN Care Manager.

Assist in implementing care plans by scheduling appointments and monitoring biometric data.

Provide and reinforce education to patients regarding their health conditions and care plan.

Complete home visits and maintain frequent contact with patients to ensure they are receiving appropriate care.

Connect patients with community resources to support their health and wellness needs.

Attend care team rounds and provide updates on patient progress.
