LPN – Case Manager

Schedule

Monday – Friday, 8:00 a.m. – 4:30 p.m.

Assignment Details

Full Time, 40 guaranteed hours per week. This is a mobile position requiring travel, including potential home visits, hospital visits, and patient office visits.

Requirements

• LPN license required
• 1 year of experience in Home Health or Mobile Case Management preferred
• BLS certification required
• Access to reliable transportation
• Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities preferred

Responsibilities

• Support the Transitions of Care program focused on reducing 30-day readmission rates
• Assist with implementing care plans under the direction of the RN Care Manager
• Schedule appointments and monitor biometric data
• Provide and reinforce education to patients
• Engage in patient visits including home visits when necessary
• Connect patients with community resources and maintain frequent patient contact
• Document interactions in the electronic health record (EHR)
• Participate in care team rounds, providing patient updates
• Manage a caseload of approximately 25–30 patients
