LPN – Case Manager

Schedule:

Full Time, Monday–Friday, 8:00 a.m.–4:30 p.m.

Assignment Details:
Guaranteed

Hours:

40 hours per week

Requirements:

• Current LPN license
• BLS certification required
• Minimum of 1 year of experience in Home Health/Mobile Case Management
• Experience with adult/geriatric populations preferred
• Access to reliable transportation for travel during the workday

Responsibilities:

• Support the Transitions of Care program to reduce 30-day readmission rates
• Assist the RN Care Manager in implementing patient care plans
• Schedule appointments and monitor biometric data
• Provide education and reinforce instructions to patients
• Conduct home visits and connect patients with community resources
• Document patient interactions in the electronic health record
• Manage a monthly rolling caseload of approximately 25–30 patients
