LPN – Case Manager

Schedule:

Full Time, Monday–Friday, 8:00 a.m.–4:30 p.m.

Hours:

40 per week

Requirements:

• Licensed Practical Nurse (LPN) license required
• 1 year of experience in Home Health or Mobile Case Management
• BLS certification required
• Access to reliable transportation for travel during work hours
• Clinical knowledge or experience with CHF, COPD, and complex comorbidities preferred

Responsibilities:

• Support the Transitions of Care program aimed at reducing 30-day readmission rates
• Assist the RN Care Manager in implementing patient care plans
• Perform appointment scheduling and biometric data monitoring
• Provide and reinforce patient education
• Attend patient visits and perform home visits as needed
• Connect patients with community resources
• Maintain regular contact with patients and provide updates during care team rounds
• Manage a caseload of approximately 25–30 patients
