LPN – Case Manager

Full Time, Monday–Friday, 8:00 a.m.–4:30 p.m.

Hours:

40 hours/week. This is a mobile position requiring travel for home visits, hospital visits, and patient office visits.

Current LPN license.

1 year of experience in home health or mobile case management.

BLS certification required.

Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities preferred.

Access to reliable transportation.

Support the Transitions of Care program and assist with care plan implementation.

Appointment scheduling and biometric data monitoring.

Provide patient education and connect patients with community resources.

Conduct home visits and maintain regular patient contact.

Document in the electronic health record (EHR).

Participate in multidisciplinary care team rounds and manage a monthly caseload of approximately 25–30 patients.
