Location: City of Rochester
Permanency Specialist II Youth – Institute for Family Connections
Join the Institute for Family Connections® to work with youth and families participating in Hillside’s Residential Treatment Services on the Monroe Avenue Campus. This is a Full‑Time, 40‑hour-per‑week position with a flexible schedule. Responsibilities include individualized permanency planning, Family Finding services, collaboration with County and community partners, clinicians and treatment teams, and building supportive networks for each youth.
Essential Job Functions- Implement the Family Finding and/or Child‑focused Recruitment Model with assigned youth.
- Facilitate and coordinate Permanency Round tables for referred youth.
- Engage youth, caregivers, relatives, fictive kin, and other appropriate adults to increase connections.
- Support teams in determining the type of permanency intervention needed.
- Educate youth, families, providers, and strategic partners (e.g., DSS, OCFS) about the impact of Family Finding and Wendy’s Wonderful Kids permanency services.
- Participate and provide expertise in Service Plan Reviews, Treatment Conferences, and Benchmark team meetings related to permanency.
- Develop, maintain and update required documentation in a timely manner using effective written and oral communication.
- Perform other duties as assigned.
- Associate’s degree in human services or related field with minimum 2 years relevant experience; or
- Bachelor’s degree in social work, psychology, or human services required; or
- Bachelor’s degree in any field with minimum 2 years relevant experience required.
- Unrestricted, valid NYS driver’s license for at least 1 year with a clean driving record and insurance coverage meeting agency standards.

- Implement and facilitate Permanency Round Table and Model meetings.

- Variable schedule, including evenings and weekends.
- Sitting (up to 6 hours), occasional standing (up to 2 hours), walking (up to 2.5 hours), driving (several times a week up to 6 hours).
- Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching, running, twisting (up to 2.5 hours).
- Lifting up to 10–15 lbs weekly on a daily basis.
- Manual dexterity required (using both hands).
- Occasional exposure to dust, fumes, gases, chemicals, or smoke.
- Ability to change positions as needed.
- Staff may be required to physically restrain clients weighing 50–300 lbs for up to 15 minutes in emergencies.
$21.50 minimum pay rate, $31.00 maximum pay rate based on experience; may include Regional and/or Department premium.
