Location / EmploymentLocation: Community Based, 1099 Jay Street Home Location.
Department: Children, Youth, and Family Services.
Employment Type: Full Time.
Schedule: Monday - Friday daytime.
Salary: $22.50 - $24.00 / hour.
The Permanency Resource Center Case Manager provides supportive services in the home or community. The position connects families to resources such as parent training, peer support, mentoring, navigation for cross-system needs, therapeutic service referrals, counseling, respite activities, and other supports to help families address issues as they arise. The Case Manager exercises casework assessment skills and independent judgment, decision making, and creativity to assist individual/family growth and development while enhancing the stability of their social network.
Essential Duties and Responsibilities- Provides therapeutic services to families primarily through home visits, including individual, couple, family, grandparent, or other relative/child counseling; parent training/education; advocacy as needed.
- Provides case management, information, and referral to additional supportive services such as counseling, education, and advocacy.
- Serves as group co‑facilitator for adoptive and kinship families using curriculum to strengthen familial relationships with a trauma‑informed approach.
- Develops and implements community activities for intergenerational, adoptive families.
- Works collaboratively with Monroe, Wayne, Livingston, and Steuben Counties, NYS Kinship Navigator, and NYS Office of Children and Family Services.
- Collaborates with agencies and local district offices to build a supportive community network and educate families about available services.
- Attends local community events to educate the community and obtain program referrals.
- Stays up‑to‑date on community resources and services that benefit parents and children in the program.
- Regularly meets with program supervisor to evaluate and implement professional growth and performance.
- Informs supervisor in a timely manner about significant client events.
- Participates in consistent supervision, ongoing staff training, and program development.
- Initiates professional development through current literature, program activity development, and continuing education.
- Completes required record documentation per program policies.
- Documents significant interventions in case records.
- Completes strength‑based assessments and needs analysis with client(s).
- Creates client action plans, conducts case reviews, and participates in client‑based meetings.
- Coordinates service delivery and completes required pre/post‑test evaluations and screenings.
- Completes all documentation within required timelines.
- Participates in agency meetings and trainings.
- Actively pursues development of professional competencies through supervision, in‑service training, conferences, and workshops.
- Supports Agency’s Mission, Vision, Values, and Strategic Plan, including core competency initiatives.
