Outreach Specialist

Outreach Specialist – Coordinated Care Services, Inc. (CCSI)

Full‑time, Rochester, NY – On site, 40 hours (12pm to 8pm)

• Develop relationships with at‑risk youth, young adults, and their families.
• Conduct neighborhood canvassing, especially in high‑risk areas and after shootings.
• Facilitate mediations (The Hub).
• Partner with RCSD schools to mentor and support youth.
• Table at community events.
• Collaborate with other organizations doing community outreach.
• Maintain and complete documentation and reporting.
• Attend daily outreach debriefing meetings.
• Provide backup support for Rise Up Rochester programming.
• Perform other duties as assigned.

• High school diploma/GED and 2 years work experience in human services, or equivalent education and work experience (see CCSI’s equivalency chart).
• Training in solution‑focused, trauma‑informed care, restorative practice, and conflict resolution.

• Valid Class D New York State driver’s license and access to a motor vehicle.
• Satisfactory completion of required background screening.
• Flexibility to work Saturdays and Sundays for neighborhood events.
• Respect the confidential nature of all CCSI and customer information, including PHI and PII, in accordance with CCSI’s Code of Conduct and Privacy and Security Policies.
