Full-Time; 37.5 hours per week (M – F 8:30a-5:00p; some nights and weekends)

Pay Rate: $18.00/hr

Summary

The After Care Manager provides ongoing support and advocacy for individuals and families who have secured affordable housing. This role is key in ensuring clients maintain stable housing long-term by offering assistance in landlord mediation, resolving tenant issues, and helping clients navigate challenges that could threaten their housing stability. The After Care Manager works closely with clients to ensure they have the resources and support needed to thrive in their housing situation, promoting both physical and emotional well-being.

Job Duties
  • Provide ongoing case management to individuals and families who have secured housing through affordable programs, assisting them in maintaining stable living situations.
  • Serve as the primary point of contact for clients experiencing housing difficulties, including financial challenges, eviction risks, or interpersonal conflicts with landlords or neighbors.
  • Offer counseling and guidance to clients regarding tenant rights and responsibilities, including how to address issues with landlords, maintenance, and lease violations.
  • Act as a mediator between tenants and landlords when disputes or issues arise, working to resolve conflicts and prevent eviction.
  • Advocate on behalf of tenants to secure reasonable accommodations or resolve tenancy-related challenges, such as rent payment issues, lease violations, or property maintenance concerns.
  • Assess situations quickly, identifying potential risks to housing stability, and create an action plan for intervention and resolution.
  • Maintain accurate and confidential client records, including referral documentation and track outcomes using provided reporting tools.
  • Maintain a comprehensive knowledge of local, state, and federal affordable housing programs, including public housing, Section 8, etc.
  • Maintain relationships with local housing authorities, landlords, and property owners to stay informed about housing policy, local resources and best practices in housing stabilization to ensure clients' needs are met.
  • Screen clients for other social determinants of health and connect clients to other community resources, such as financial assistance programs, utility assistance, health home care management, legal, and health-related support.
  • Perform other duties as assigned.
Knowledge, Skill and Abilities
  • Knowledge of affordable housing programs, tenant rights, and fair housing laws is highly desirable.
  • Ability to meet clients at rental housing units, as needed.
  • Demonstrate cultural competence and the ability to interact effectively with diverse populations, including low-income individuals, families, seniors, and people experiencing homelessness or housing instability.
  • Contribute to a creative, collaborative, and respectful environment that promotes teamwork and an anti-racist, community-centered mindset.
  • Demonstrate commitment to confidentiality, ethical standards, and adherence to company policies and procedures in all professional interactions and handling of sensitive information.
  • Strong communication skills, both verbal and written, with the ability to explain complex housing processes in an easy-to-understand manner.
  • Excellent organizational and time-management skills, with the ability to effectively prioritize tasks and meet deadlines.
  • Excellent problem-solving skills and the ability to de-escalate tense situations or conflicts.
Physical Elements
  • Must be able to physically perform stooping, kneeling, walking, lifting, grasping, pulling, standing, talking, hearing.
  • Must be able to perform light work exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
Qualifications
  • High School diploma, AA/AS in the field of Human Services, and/or lived experience in housing navigation, family and/or individual case management, social services, or a related field preferred.
  • A valid driver’s license (if required for local travel) and reliable transportation may be necessary.
