Apply for the Case Manager - Women's Shelter (Temporary to Permanent
-Full Time) role at The Salvation Army USA Eastern Territory. This position is 35 hours per week (Mon‑Fri 8:30 am – 4:00 pm, with one day per week scheduled for a later start to see clients in the early evening).

Pay Range

$22.00/hr – $22.00/hr (actual pay based on skills and experience).

Responsibilities
  • Assess and identify clients' needs. Advocate for clients to ensure that services of referral are accessible and provided to clients.
  • Develop an individualized service plan for each resident and ensure periodic review of goals.
  • Provide supportive and crisis counseling and guidance both individually and to groups.
  • Provide information and refer clients to relevant community social services for medical, mental health, substance abuse, educational, vocational, public assistance, etc.
  • Maintain all records and documents relative to the interview/counseling meeting and subsequent progress/status reports in accordance with agency and contract requirements.
  • Develop and maintain effective working relationships with other community social service providers (e.g., Dpt. of Human Services, Veteran’s Administration, etc.) to enhance the quality of services to clients. Represent the program in the community as requested and as approved.
  • Report observations to supervisor on trends of the homeless, their service needs, effectiveness of referrals, etc.
  • Participate in weekly meetings with Senior Resident Assistant and Program Manager.
  • Maintenance of HMIS data entry (intakes, transfers, discharges, etc.).
  • Compile monthly statistics and prepare required reports in a timely manner.
  • Collaborate with staff for case management and programmatic services.
  • Share 24 hr. on‑call responsibilities with the Program Manager and Senior Resident Assistant once every three weeks.
  • Attend staff meetings, in‑service conferences, seminars, training programs, and other agency informational meetings as required.
  • Complete all mandatory training (in‑person and online) in a timely manner.
  • Share office coverage at least 1–2 x a week during lunch time, if needed.
  • Other duties as assigned.
Qualifications
  • Bachelor’s degree in human services preferred (Social Work, Psychology, Sociology, Criminal Justice, etc.) with 2 years case management experience required.
  • Requires experience working with and providing advocacy for vulnerable populations, as well as an understanding of barriers faced by those who are homeless.
  • Concern and sensitivity toward vulnerable populations. Knowledge of social service resources.
  • Must be proficient with Microsoft Office suite of applications and other standard data collection systems.
  • Ability to collaborate effectively with external service providers.
  • Certification in Standard First Aid and CPR.
  • Hepatitis vaccination or declination (within ten days of hire).
  • Must possess strong organizational abilities and attention to detail. Excellent communication and problem‑solving skills.
  • Valid New York State Driver License for transport of residents, if necessary.
