Overview
We are seeking a skilled and qualified Primary Care Occupational Therapist to service our patients throughout the Rock Hill area. An Occupational Therapist will play an essential role in helping patients achieve independence and improve their ability to perform daily activities. You will work closely with Certified Occupational Therapy Assistants (COTAs) to deliver patient‑centered care, perform detailed assessments, develop customized treatment plans, and supervise the implementation of these plans by COTAs.
This is a full‑time, salary‑based position with an opportunity to receive a monthly bonus. You will work 8‑hour days (8:00 AM–5:00 PM), Monday‑Friday, and conduct visits in homes, assisted living facilities, and inpatient long‑term facilities.

Responsibilities- Conduct comprehensive evaluations of patients’ physical, emotional, and social conditions.
- Identify patient needs and set realistic goals for therapy.
- Develop individualized treatment plans aimed at improving patients’ daily living and working skills.
- Adapt treatment plans as necessary based on patient progress and feedback.
- Work with COTAs, providing guidance and supervision.
- Review and monitor the implementation of treatment plans by COTAs.
- Use telehealth tools to collaborate with COTAs and other healthcare professionals.
- Participate in care team meetings as necessary.
- Communicate effectively with management regarding patient outcomes and program improvements.
- Facilitate visits with providers (MD, NP, PA) to ensure timely communication, coordination of care, and alignment on patient treatment plans.
- Serve as a liaison between therapy services and providers by sharing updates, raising concerns, and reinforcing care‑plan goals.
- Integrate provider input into therapy interventions and document appropriately to maintain continuity of care.
- Recognize that facilitating provider visits is a core responsibility of the role.
- Meet staff efficiency expectations by performing consultations, assessments, and other occupational therapy services efficiently and effectively.
- Appropriately and accurately document and log care‑management activities; keep patients’ care plans up to date.
- Educate patients and their families on the goals of therapy and how to achieve them.
- Provide training on the use of adaptive equipment and strategies for improving daily function.
- Provide support and encouragement to patients to promote adherence to treatment plans.
- Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress notes.
- Report any changes in patient condition or treatment plans to the appropriate members of the care team.
- Ensure all activities comply with federal and state regulations, as well as company protocols.
- Master’s or Doctoral degree in Occupational Therapy from an accredited program.
- Current state license to practice as an Occupational Therapist in good standing.
- Minimum of two (2) years of clinical experience, preferably in a home‑health or outpatient setting.
- Strong skills in occupational therapy treatment techniques and patient care.
- Proficiency in using telehealth platforms and electronic health records (EHR).
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational and time‑management skills, with the ability to multitask and prioritize responsibilities effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Ability to manage and demonstrate effective leadership skills.
- Team player capable of managing multiple responsibilities and demonstrating sound judgment.
- Must be able to work flexible hours and travel between offices, facilities, etc.; must be a licensed driver with an insured automobile that is in good working order.
