How You'll Make a Difference

We are seeking a Nurse Practitioner who demonstrates clinical expertise with acute and chronic conditions. The position is recognized as an Advanced Practice Registered Nurse and Independent Licensed Practitioner by the Wyoming State Board of Nursing, providing care within the scope of practice as defined by state law, licensing, and regulations.

Essential Duties and Responsibilities
  • Perform in‑home history and physical assessments that are accurate, relevant, and appropriate for the patient.
  • Demonstrate knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.
  • Conduct thorough review of member records prior to visits, using critical thinking to interpret diagnostic studies and develop diagnoses and plans.
  • Develop, implement, evaluate, and modify treatment plans as needed.
  • Make medication recommendations and adjustments.
  • Maintain accurate, complete, and timely documentation in the electronic health record, including correct diagnosis codes and status of all conditions.
  • Understand key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Category (HCC) models within 30 days of hire.
  • Understand key elements of Quality STAR/HEDIS measures within 30 days of hire.
  • Participate in telephonic on‑call coverage after hours.
Qualifications and Skills
  • Current and valid State of Wyoming RN license required.
  • Bachelor’s degree in Nursing and Wyoming Advanced Practice Nursing license required;
    Master’s degree preferred.
  • Board certification from a nationally recognized certifying body for Nurse Practitioners required.
  • Unrestricted prescribing and dispensing privileges with DEA licensure required.
  • Minimum of 1 year experience in an ambulatory or community care setting, or equivalent clinical experience emphasizing assessment skills.
  • Experience working with adults, older adults, and geriatric populations preferred.
  • Experience and understanding of population health management preferred.
  • Strong problem‑solving and prioritization skills for dynamic patient conditions.
  • Flexibility and adaptability to shift work assignments based on organizational needs.
  • Driving is a requirement; current driver’s license and proof of insurance required.
