Position SummaryIn this role, you will be dedicated to providing post‑crisis stabilization and facilitating connections to ongoing care tailored to each client’s needs. Responsibilities include conducting formal assessments, identifying treatment requirements, and developing comprehensive safety plans in collaboration with clients and their families. The role also involves working closely with other programs, such as Mobile Crisis Response (MCR), to ensure clients receive the necessary support and resources.

$3,000 for non‑licensed applicants, $4,000 for 1st or 2nd level license holders.
Qualifications- Bachelor’s degree in Psychology, Counseling, Social Work, or related field, or equivalent experience and training.
- Must qualify at a minimum as a MHP per Illinois Department of Mental Health Rule 132.
- Excellent written and oral communication skills.
- Ability to quickly establish rapport with clients in crisis.
- Computer proficiency required.
- Ability to navigate and complete documentation in the electronic health record.
- Ability to accurately complete required documentation within prescribed time frames.
- Self‑starter able to work independently and collaboratively within a team.
- Ability to manage multiple deadlines.
- Valid driver’s license and ability to meet agency insurance requirements.
- Ability to transport clients as needed.
- Belief in the mission and vision of Rosecrance.
- Work scheduled hours and any additional hours deemed necessary by leadership.
- Participate in the on‑call system as required.
- Provide crisis stabilization services to meet the needs of clients in community settings.
- Respond to behavioral health crises with appropriate crisis intervention strategies.
- Observe clients in their natural environment and provide coaching on effective use of their crisis safety plan.
- Model positive coping skills and response patterns for clients and their families.
- Redirect client behaviors when they begin to escape and educate clients on reducing environmental stressors.
- Complete required assessments and documentation to link clients to longer‑term care programs.
- Utilize Electronic Health Record to open clients into services and audit charts for necessary documentation.
- Maintain a caseload of crisis clients with the goal of linking them to long‑term care.
- Ensure crisis program fidelities are met regarding discharge planning, MCO communication, and client staffing through monitoring documentation and performing related tasks.
- Communicate effectively with internal and external stakeholders to coordinate care and ensure accurate discharge planning.
- Serve as a role model for staff, clients, and customers, demonstrating positive guest relations in representing Rosecrance.
- Understand and comply with the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.
- Perform all responsibilities in compliance with the mission, vision, values, and expectations of Rosecrance.
- Deliver exceptional customer service consistently to every customer.
- Assume other related responsibilities as assigned by leadership.
- Monday - Friday 10:00 am – 6:30 pm (40 hours per week).
Onsite – Rosecrance Ware Center, Rockford, IL.
Compensation- Base Pay:
Starting at $50,835.20 per year (pay based on education, experience, and credentials).
