Triage & Chronic Care Nurse Manager – Rockford Gastroenterology Associates, Ltd.

This position reports to the Clinical Administrator and is part of the Triage department. The manager oversees CCM nursing and clinical support staff, coordinating complex patient care and ensuring seamless transitions between healthcare settings.

Responsibilities
  • Supports the mission and philosophy of Rockford Gastroenterology Associates.
  • Supervises staff in day‑to‑day functions, including training and development.
  • Assists the team during staffing shortages and workload fluctuations.
  • Works as part of the healthcare team to assess patient needs, plan and modify care, and implement interventions.
  • Collaborates with the Clinical Administrator on hiring, termination, and employee evaluations.
  • Monitors orientation of all new personnel and schedules in‑service training.
  • Conducts skill verifications with the Clinical Administrator.
  • Attends Joint GI Lab Meetings and the Quality Committee.
  • Prepares staff work schedules.
  • Meets with sales representatives as it relates to nursing activities.
  • Evaluates the quality of care provided in Triage and CCM through observation, patient interviews, and other tools.
  • Conducts yearly performance evaluations and goal reviews with each staff member.
  • Initiates coaching or corrective action when necessary, following organizational policy.
  • Directs staff in developing therapeutic care plans using the nursing process.
  • Oversees the patient’s ongoing educational process.
  • Works with providers to resolve patient care issues.
  • Collaborates with other departments.
  • Responds to drills and reviews procedures with staff to ensure knowledge retention.

Qualifications
  • Current Illinois State Registered Nurse licensure.
  • Graduate of an accredited nursing program.
  • Current BLS and ACLS certification.
  • Experience in gastroenterology preferred.
  • Experience in directing, teaching, and supervising personnel.
