Position Overview: Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.

Qualifications:

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driver's License
• Valid Auto Insurance
• Minimum 1 year home health experience
• Proficiency in Well Sky Kinnser EMR
• Strong clinical assessment and communication skills
• Compassionate and patient-centered approach

Pay: $45 to $50 per hour OR $80,000.00 - $ per year

Schedule: Monday to Friday, Weekends as needed
