In this critical role, you’ll guide patients through the PD process, offering hands‑on training, support, and clinical expertise that transforms lives.

• Provide one-on-one and group training to patients initiating peritoneal dialysis.
• Educate patients and caregivers on home dialysis procedures, infection prevention, and equipment use.
• Monitor patients’ progress and collaborate with interdisciplinary care teams.
• Ensure compliance with clinical policies and regulatory standards.
• Document patient interactions and training outcomes accurately and promptly.

• Active Registered Nurse (RN) license in the state of Illinois.
• Minimum of 6 months of hands‑on peritoneal dialysis experience
