We’re hiring an LPN to serve patients in Rockford and nearby communities (within ~30 minutes).

• Visiting patients in their homes (30‑45 minutes per visit) and delivering compassionate, patient‑first care.
• Assessing physical, emotional, and social needs, and following individualized care plans.
• Providing skilled nursing care — including wound care, Foley/G‑tube care, and IV tasks within IL LPN scope & agency policy.
• Teaching patients and families about medications, conditions, and ways to stay healthy at home.
• Working closely with physicians, therapists, and your care team.
• Documenting care in our EMR and updating the RN/clinical leadership when patient needs change.

• Active Illinois LPN license;
  Associate’s in Nursing or equivalent.
• Home Health experience required; comfortable using EMR.
• Familiarity with OASIS is a plus (RNs lead OASIS assessments).
• Strong communication skills and the ability to adapt to different home environments.
• Understanding of Home Health guidelines and regulations.

• Paid, tailored training (~2 weeks) in Arlington Heights to set you up for success.
