Make a lasting difference as a Hospice Registered Nurse Case Manager (RNCM)! Sanctuary Hospice is seeking a Full‑Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.
Schedule: Monday – Friday, 8 am – 4:30 pm and On Call As Needed.
Territory: Rockford and surrounding counties – Winnebago, Boone, McHenry, Dekalb, Ogle, Stephenson, and Lee.

- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
- Partner closely with our Clinical Director to assess eligibility and suitability of clients for specialized hospice services.
- Provide comfort and support to patients, ensuring their comfort and quality of life remain paramount.
- Deliver top‑notch nursing care rooted in proven principles and techniques, easing pain and bringing solace.
- Educate and empower patients and families through personalized care plans.
- Share call duties as part of the team’s commitment to being there when it matters most.
- Current, unencumbered license as a Registered Nurse.
- Minimum one year of RN experience in a medical‑surgical/acute care setting; hospice experience preferred.
- Computer proficiency, including EMR background.
- Current CPR certification if required by state.
- Valid driver’s license, vehicle insurance, and dependable transportation.
$89,000 – $96,000 per year (Average Pay Range). The pay range listed is a general guideline and is not a fixed offer. Final compensation will be determined based on experience, qualifications, and specific responsibilities and will be discussed at the conclusion of the interview process.
