Mercyhealth is seeking a Registered Nurse, Patient Navigator, to coordinate and facilitate patient care, ensuring appropriate service utilization and better outcomes in Rockford, IL. Schedule includes Monday–Friday with limited on-call, dayshift, and a focus on transitions and discharge planning.
The role requires BSN (or enrollment in a BSN completion program), active RN license in Illinois, and knowledge of Allscripts Care Management and Epic EMR. Weekend/holiday on-call may be required.
