Job SummaryManages assigned caseload of Supported Housing recipients.
Duties & Responsibilities- Develop and promote appropriate services for all clients/tenants. Provide services or connect tenants to job training/placement, legal services, budgeting, entitlements, behavioral or physical health services, eviction prevention programs to support tenants in maintaining their housing.
- Provide recipients with information about support services and assistance.
- Communicate and collaborate with other community agencies/departments regarding clients.
- Oversee documentation of recipients, i.e., admission notes, service plans, progress notes, billing timely entered into CAIRS, all documentation properly recorded in clients' case records.
- Ensure clinical records and related source documents are maintained in accordance with OMH and agency requirements.
- Assist recipients in attaining Federal Section 8 or other subsidies.
- Ensure all clients entering the program are provided with the Residency Agreement and all program procedures.
- Ensure tenants are aware of tenancy obligations through tenant orientation, handbook, and individual meetings with tenants.
- Identify households in distress and work in collaboration with property coordination to prevent lease compliance issues and eviction.
- Meet regularly with property coordination staff to review tenants in arrears and implement prevention programs including rent payment plans.
- Work with supervisors to apply evidence-based practices into work; encourage focus on employment, reduction of reliance on benefits, and income enhancement to ensure housing stability.
- Provide other related housing assistance as necessary.
- Work independently with some supervision.
- Visit individuals a minimum of once per month, or as necessary.
- Maintain accurate timekeeping in a timely fashion.
- Keep informed of current trends in the Mental Health Field.
- Attend staff meetings as required by the Program Supervisor.
- Contribute to the safety and therapeutic environment of the program.
- Ensure safety and health standards are maintained.
- Be available to clients by phone in emergencies or as needed.
- Bachelor’s degree in human services.
- One (1) year of experience working with individuals with mental health conditions and people experiencing mental health challenges is required.
- Valid, non-restricted NYS Driver’s License.
While performing the duties of this job, the employee frequently must stand or sit for long periods, occasionally sit and stoop, kneel, crouch, or crawl, and occasionally lift and/or move up to 25 pounds.
