Position: Home Care Physical Therapist (PT) Per Visit VNA

Home Care Physical Therapist (PT) Per Visit VNA

Under the direction of the Clinical Manager, evaluates, plans, and administers medically prescribed physical therapy treatments to patients in their home.

Essential Functions
  • Initial patient assessment:
    Performs comprehensive physical, psycho‑social, and safety assessment, identifies functional problems and establishes interventions and goals.
  • Initial assessment documentation:
    Completes thorough documentation, performs medical reconciliation, and synchronizes records with the agency’s guidelines.
  • Revisits:
    Provides skilled PT treatment in the home, reassesses patient status on every visit, modifies treatment plans as needed, orders equipment, monitors pain and vital signs, and completes Medicare Functional Reassessments.
  • Coordination of patient care:
    Coordinates care with clinical team members, informs MD of status changes, and obtains authorization for managed‑care patients according to agency guidelines.
  • Recertifications:
    Completes agency recertifications within 5 days of the certification period, updates orders, and communicates with relevant disciplines.
  • Discharges:
    Documents discharge planning, informs all disciplines and MD, provides discharge summaries, and ensures completion of all required paperwork.
  • Technology:
    Demonstrates proficiency in the automated clinical record system by completing the required education program and using the software daily.
  • Safety:
    Fosters a culture of safety, maintains personal ownership, and follows infection‑control procedures during home visits.
  • Scheduling:
    Maintains an up‑to‑date schedule in the computerized system, submits the next week’s schedule by Friday morning, and keeps patients informed about their individualized visit dates.
  • Professional development:
    Maintains and enhances knowledge, attends in‑services, and participates in committees or team projects as needed.
  • Technology and learning:
    Pursues continued learning, embraces new technology, and uses procedures that improve efficiency and effectiveness.
Job Requirements
  • Minimum Education
    : BS degree in Physical Therapy preferred.
  • Experience
    : 1–2 years of clinical experience with a varied patient population.
  • Licenses/Registrations
    :
    Licensed Physical Therapist in the State of Massachusetts.
  • Salary
    : $41.00 – $67.00 per hour.
  • Hours
    :
    Part‑time; budgeted hours: 0 (as part of the VNA agreement).
