The Social Care Screener/Navigator (SCN) plays a critical role in improving the health of individuals and communities by screening for Health-Related Social Needs (HRSNs), providing health education, and connecting individuals to appropriate services and community resources. This position serves as a liaison between the healthcare system, social service providers, and community-based organizations to coordinate care, improve access, and promote culturally competent service delivery.
Duties & ResponsibilitiesConduct outreach and engage Medicaid Members to assess HRSNs and connect them to appropriate services that align with their preferences, limitations, or disabilities. Perform eligibility assessments, manage referrals, coordinate care, and provide health education. Confirm with Members whether referrals were accessed and needs were met. Interview participants to gather basic demographic, medical, and income information to determine program eligibility. Assist with benefit program applications as applicable and connect Members to clinical care management support.
Develop Social Care Plans for eligible Members, including a summary of needs, eligibility status, and referred services. Monitor and respond to Member communications via phone and through the Care Compass Collaborative website. Establish and maintain collaborative relationships with community-based organizations and service providers.
High school diploma or equivalent required. Minimum of two (2) years of experience in healthcare, community health, or social services required.
Physical RequirementsMust be able to communicate clearly and effectively. Regularly required to sit, stand, walk for extended periods, and occasionally kneel, crouch, or climb stairs. Prolonged periods of sitting at a desk and working on a computer. May be required to lift up to 25 pounds or more. Required to commute to other agency locations as needed.
