The APRN is a clinical role that will provide exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This role is a safety-sensitive position as it involves direct patient care and administering medication.
The incumbent will report to the Regional Manager (or similar role) and will work closely with other healthcare professionals to provide quality care to patients, collaborating with other clinical teams to support team function.

- Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine; know limitations and seek consult when advisable; keep abreast of current technologies and industry standards
- Under the supervision of a physician, provide care for patients with acute and chronic illness encountered in the older adult patient
- Under the supervision of a physician, accountable for patient care and outcomes; seek consultation from specialists when needed while staying involved in the detailed care of the patient
- Engage with hospitalists whenever a patient is in the hospital
- Meet all clinical obligations of the medical practice:
Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc - Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines
- Follow all internal policies and procedures
- Complete operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation
- Maintain complete and detailed records in the EMR of patients’ health care plans and prognosis, completed within 48 hours of each visit
- Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability
- Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosis
- Interpret data and diagnostic results for an appropriate course of action in collaboration with supervising physician
- Exhibit compassion, quality, loyalty, and conscientiousness
- Establish and maintain effective working relationships with patients, families, and team members
- Document continuing education per licensure and certification requirements
- Ensure patients are seen every other visit by the physician
- Maintain patient confidentiality in accordance with policy and HIPAA requirements
- Enact proper chain of command for patient complaints
- Additional duties as assigned
- Graduate of an accredited school of nursing
- Certified as a nurse practitioner with current state licensure
- At least 1 year of professional nursing experience with 1-3 years as a practicing primary care nurse practitioner
- Prior experience working in an electronic medical records system
- Provide 5 Star service in accordance with the Better Health Group's mission
- Skilled in traditional nursing disciplines of patient assessment and management
- ACLS/BLS certification required as mandated by state
- Knowledge of rationale of appropriate patient care
- Ability to handle emergency situations calmly and effectively
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to present information and respond to questions from diverse groups
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Excellent written and verbal communication, teaching ability, and good telephone skills
- Working knowledge with Google Suite and Microsoft Office for real-time collaboration
- Full range of body motion; ability to lift and move patients and medical equipment as needed
- Requires standing, walking, bending, kneeling, and reaching for prolonged periods
- Requires ability to lift/carry up to 15 lbs
- Requires corrected vision and hearing to normal range
