Responsible for assessing patients and families who request admission to Montgomery Hospice. During assigned visits, the Admissions RN provides a comprehensive assessment of patients to be admitted, initiates the plan of care and performs other nursing tasks as needed. Must be willing to work in both Montgomery and Prince George’s counties.
Duties and Responsibilities- Accurately assess patients and family needs.
- Identify and recommend medical equipment and supplies that assist in maintaining patient comfort and caregivers’ ability to provide care.
- Identify the need for additional services, including nutritional assessment, physical therapy, occupational therapy and speech therapy, and initiate appropriate referrals.
- Assess need for and educate caregivers on safety and emergency preparedness, including safe use of medical equipment.
- Convey physician’s requests to appropriate staff on appropriate forms.
- Current state of Maryland Nursing License; CHPN preferred.
- Graduation from an accredited nursing program; B.S.N preferred.
- Two years of recent clinical experience, preferably with hospice and/or critical care.
