• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their homes according to the active plan of care given by the physician and other appropriate personnel.
• Conduct ongoing patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate care.
• Educate family members on patient clinical care to enhance positive outcomes.
