The Sgt. Johnow Healthcare Center at the Connecticut Veterans Affairs is a Skilled Nursing Facility licensed by the Connecticut Department of Public Health. The Levitow Healthcare Center takes pride in being certified by the Centers for Medicare & Medicaid Services (CMS), achieving their highest 5-star rating. This exceptional distinction indicates the Levitow Healthcare Center is considered much above average in quality by CMS.
The Levitow Healthcare Center is also certified annually as a State Veterans Home by the Federal Veterans Administration.

The Sgt. John L. Levitow Healthcare Center - Location:

In the Departments of Developmental Services, Emergency Services and Public Protection and Veterans Affairs this class is accountable for providing professional social work services and/or programs required by communities and/or individuals to promote and sustain social functioning.
Examples of Duties- Conducts in depth interviews with individuals, their families, medical and psychiatric professionals and other resource individuals in order to obtain complete individual histories;
- Determines, researches, reviews and assesses individual and/or community needs and develops treatment or development plans to address them;
- Contracts with outside agencies to assist in treatment of complex individual and/or community problems;
- Assesses individual functioning level to determine suitability of community residential and or employment placement;
- Makes placements when appropriate and provides case management services for individuals and/or communities;
- Conducts individual, group and family counseling and therapy sessions;
- Provides crisis intervention services to individuals and/or communities in emergency situations;
- Provides training, outreach and consultation to individuals, colleagues, staff, families and community organizations on relevant issues;
- Conducts community outreach and follows up accordingly to meet identified needs;
- Serves as individual advocate or agency representative in legal proceedings;
- May function as team leader, project manager, social representative and/or Qualified Intellectual Disability Professional on interdisciplinary team;
- May assist in development of institutional and/or community policies, strategic plans, immediate response plans and programs;
- May recruit, evaluate and license private community training homes;
- May participate in development and implementation of individual education or program plans for individuals and/or communities;
- May serve as a liaison between multiple stakeholders for program implementation and development;
- Performs related duties as required.
- Considerable knowledge of
- social, psychological, economic and medical factors that influence behavior of individuals;

- Knowledge of community resources available to individuals and families;
