Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in‑house support staff. The primary focus of this role is to provide family, geriatric, or internal medicine primary‑care services in accordance with Eventus Whole Health site‑specific protocols and the standard of care for long‑term‑care facilities.
Essential Duties and ResponsibilitiesProvide primary‑care services in accordance with the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term‑care facility work.
Quality/Compliance:
Review and sign medication administration records (MARs), all primary‑care orders, lab results, narcotic prescriptions, and other orders, ensuring timely completion and documentation within specified time frames.Serve as on‑call triage provider per the on‑call triage protocol; adjust schedules as needed and adhere to the latest triage schedule.
Complete and submit billing records and documentation to Eventus Whole Health, and provide a hard copy of electronic documentation within 48 hours for inclusion in the facility’s medical record.
Maintain open, positive communication with facility staff and administration; provide verbal and written instruction or feedback regarding medications and other pertinent caregiver information.
Adhere to Eventus Whole Health standards, policies and procedures, and all legal and regulatory requirements, including HIPAA.
Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.
Masters degree in Nursing from an accredited institution and a current, valid state nursing license. All other certifications required for duties must be current and valid.
Physician Assistant pathway:
- Graduated from a physician assistant program accredited by a National Commission on Accreditation of Allied Education Programs.
- Passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
- Licensed by the state in which you reside as a Physician Assistant.
- Specialization in geriatric, family, or psychiatry desired.
