Position: Home Care Licensed Practical Nurse Role

Your position entails working as part of an interprofessional team to assess, plan, monitor, and deliver care to clients alongside a Registered Nurse Case Manager. Key Responsibilities:

• Plan and implement assessments in client care

• Deliver patient-centered care in residence and alternate settings

• Evaluate client health outcomes and adjust care plans

• Work collaboratively with other health professionals

• Maintain compliance with health service standards

Requirements:

• Graduation from an accredited practical nurse education program

• Valid LPN registration with CLHA

• Two years' experience in Home Care within the last five years

• Current BCLS - Health Care Provider certification

• Capable of managing physically demanding tasks
