Petaluma Health Center is seeking an experienced Nurse Practitioner or Physician Assistant who has adult and pediatric urgent care or emergency room experience as well as and understanding of the primary care medical home model to join our welcoming and diverse community of providers. We are considering fulltime, part-time or per diem schedules.

Summary:This provider will deliver patient-centered and culturally appropriate care to a diverse patient population and specifically manage patients’ acute conditions in both in-person and tele-health modalities including any urgent social and behavioral health concerns This provider will work closely with a collaborative team to ensure a proper bridge to the patient’s primary care medical home and avoidance of improper emergency room utilization.

Areas of Responsibility

• Provides triage, assessment, diagnoses, treatment of acute and episodic conditions and communicates with the local emergency departments, hospitals and outside providers when appropriate
• Interprets on-site diagnostic testing (laboratory, x-ray, ECG etc) for diagnosis and management of patient conditions
• Performs a variety of procedures necessary for urgent treatment of patients which include but are not limited to the following: incision and drainage, suturing, splinting, joint injection, wound care, gynecologic procedures. Point of Care ultrasound skills desired but not required.
• Provides short-term management and bridging of care for chronic diseases (e.g., diabetes, high blood pressure) and short-term management and bridging of care for preventive care for adults and children including screening and immunization
• Performs telemedicine, video and in person visits depending on the specific needs of the patient
