Position Overview
The Community Health Worker (CHW) serves as the frontline public health worker addressing the healthcare coverage needs of Four Rivers patients and the broader community. The CHW provides comprehensive evaluation of health coverage options and assists with Medicaid, Medicare, Marketplace, and other insurance enrollment.
SupervisionNon-supervisory; reports directly to the Care Management Manager.
Responsibilities- Help patients navigate community services and adopt healthy behaviors.
- Assist eligible clients in applying for publicly sponsored health care insurance plans; provide follow‑up throughout the approval process.
- Conduct community outreach to promote individual and community health.
- Provide information on available resources and offer social support and informal counseling.
- Advocate for individuals and community health needs; act as liaison between communities, individuals, and service providers.
- Collect data to identify community health needs and conduct continuous process improvement as part of the quality assurance program.
- Establish trusting relationships with community partners, patients, and families while providing general support and encouragement.
- Contact prospective program participants by mail, telephone, or in person to inform them of available services.
- Assess client eligibility for services such as financial assistance, insurance, and other supportive programs.
- Refer clients to community resources and agencies to meet identified needs.
- Attend community meetings and health fairs to understand community issues and provide information to community members.
- Help clients understand health conditions, develop strategies to improve them, and provide informal counseling, support, and follow‑up procedures.
- Improve community residents’ ability to communicate effectively with health care providers; reduce cultural and socio‑economic barriers.
- Distribute posters, flyers, and brochures appropriate for the target population.
- Utilize electronic health record, spreadsheets, word processing, and other software to format documents, spreadsheets, brochures, flyers, newsletters, training materials, and other documents.
- Enter data and produce reports as required under the Community Health Worker contract and the Primary Care Health Home program.
- High school diploma or equivalent required.
- Experience with social services or public health system preferred.
