Pediatric Physical Therapist (PT) – Home Health
We are seeking candidates to service in the following areas:

Join our dedicated team to deliver pediatric home health services and build lasting relationships with patients and families, ensuring comprehensive and compassionate care.
Responsibilities- Participate in the development and periodic revision of the plan of care.
- Schedule, plan activities, and provide rehabilitative services for physical therapy.
- Maintain adequate records on all patients, including summary reports; follow agency documentation timelines as published in the Employee Handbook.
- Utilize community resources and agency personnel for proper referrals.
- Select and administer diagnostic and therapeutic techniques and materials; submit diagnostic assessments within agency timelines as published in the Employee Handbook.
- Instruct and counsel other health team personnel and family members in methods of assisting patients in improving, correcting, and adapting to disabilities.
- Supervise the PTA as outlined by the Executive Council for Physical Therapy and Physical Therapy Examiners; complete supervisory visits for patients treated by an assistant at least every 60 days.
- Attend monthly case conferences and additional meetings as requested.
- Participate in staff development activities and in-service education as requested.
- Confirm schedule at least one week prior to upcoming visits and adjust as changes occur; communicate with office staff in a timely manner.
- Notify the agency of absences due to illness, emergencies, or leave, which will affect agreed services.
- Participate in discharge planning activities and complete the physical therapy discharge summary within agency policy.
- Graduate of a physical therapy program accredited and approved by the ECPTOTE.
- Current Texas license to practice as a Physical Therapist in good standing.
- Minimum of one (1) year of clinical experience preferred; new graduates are encouraged to apply.
