Where You Will WorkThe Heart Failure Clinic involves providing specialized care to patients with chronic heart conditions, focusing on symptom management, medication optimization, and lifestyle education. The role emphasizes close monitoring, patient counseling, and collaboration with a multidisciplinary team to improve quality of life and reduce hospitalizations. It’s a dynamic environment that combines clinical expertise with compassionate support for patients and their families.
Primary Patient PopulationPatients with chronic heart conditions.
We’re Looking For- Completion of an accredited or approved program in nursing
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
- Cardiac RN experience preferred
- As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
- Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
