Physical Therapist - Coordinator

We are seeking an experienced and motivated School-Based Physical Therapist Coordinator to lead and support therapeutic services across educational programs. This role is ideal for a licensed physical therapist with strong leadership skills and a passion for working with children in a school setting. Responsibilities

• Provide physical therapy services under a physician's prescription to students with therapeutic needs.
• Conduct initial and ongoing evaluations; document results and maintain accurate progress notes.
• Develop individualized, goal-oriented treatment plans and reassess per policy timelines.
• Collaborate with interdisciplinary teams to optimize scheduling and therapeutic impact.
• Participate in staffings, student conferences, and consult with physicians as needed.
• Coordinate special projects and programs across departments and schools.
• Oversee scheduling of evaluations, referrals, and screenings.
• Establish and uphold administrative policies and procedures related to therapy services.

• Licensed Physical Therapist in Illinois.
• Minimum 5 years of experience in a school-based physical therapy setting.
• Current Child CPR and First Aid certification (or willingness to obtain).
• Experience staff preferred.
