PELC is a not-for-profit delivering urgent care services at four treatment centres in East London. The Clinical Streamer role supports urgent care services by directing patients to the appropriate clinicians, ensuring safe and efficient care.
Job responsibilities- Take clinical responsibility for patient streaming within UTC/UCC, ensuring safe redirection to the most appropriate clinician or health‑care provider.
- Work autonomously as a practitioner, maintaining professional and legal accountability under all applicable guidelines.
- Monitor the streaming queue, managing patients who deteriorate or become acutely unwell.
- Conduct focused history taking and use observations in line with local policies and audit criteria.
- Use streaming equipment competently, and advise patients on health and social matters where appropriate.
- Ensure clear, safe handover to the Emergency Department, primary‑care teams, community services, and social‑care partners.
- Maintain accurate records and documentation in accordance with PELC, NMC/HCPC, and NHS standards.
- Act as a patient advocate, supporting those at risk due to cultural or language barriers.
- Participate in performance reviews, local research, clinical audit, and the ongoing development of the Clinical Streaming role.
- Experience in paediatric or adult emergency department, or community/primary care setting.
- Nurse practitioner, emergency nurse practitioner, paramedic or physician associate qualification.
- Registered with the NMC/HCPC and current professional registration.
- Strong knowledge of primary‑care services policy and safeguarding, sepsis and emergency care guidelines.
- Minimum 2 years of demonstrable experience working in an emergency department or urgent‑care setting.
- Understanding of primary‑care service delivery and policy development.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a DBS check.
