Clinical Pharmacist

Are you a proactive Independent Prescriber looking to transform patient care at the heart of our community? We are seeking a dedicated Clinical Pharmacist to lead medicines optimisation and provide expert clinical oversight across our network.

Key Responsibilities

• Complex Clinical Reviews:
  Conduct comprehensive, person-centered medication reviews for patients with chronic diseases and complex polypharmacy. You will be the clinical lead for our most vulnerable populations, including the elderly and those residing in care homes.
• Specialist Liaison & High‑Risk Monitoring:
  Take a central role in managing shared care protocols. You will collaborate closely with other healthcare professionals to monitor and reduce the inappropriate use of high-risk medicines, ensuring patient safety is paramount.
• System Integration:
  Act as a bridge between general practice and the wider healthcare ecosystem.
• Workload & Outcomes Management:
  Proactively manage GP workload by leading on medicines-related queries and self-care initiatives, focusing on reducing medicine waste and improving long‑term patient outcomes.

• Expertise: A deep understanding of medicines optimisation and the ability to apply clinical evidence to real-world, multi-morbid patient scenarios.
• Collaborative Spirit:
  The ability to work across multidisciplinary teams (MDTs) to integrate services and provide "joined-up" healthcare.
• Patient Focus: A commitment to the "shared decision-making" model, empowering patients to take an active role in their own health.

We specialise in supplying Clinical Pharmacists to PCNs. We help to train and support pharmacists throughout their time in primary care, which can include CPPE Primary Care Pathway, IP Course and general career development.

• Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas.
• Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice.
• Providing medication reviews, and health education advice, to patients at the designated practices by face-to-face, telephone or notes-based review, as agreed with the practice.
• Providing leadership on person-centred medicines optimisation and quality improvement.
• Processing specialist treatment letters.
• Processing hospital medication discharge letters.
• Acting as a key contact in the practices, for all medication related queries.
• Supporting improvements in clinical care, through practice-based audits and implementing change.

• GPhC Registration
• Independent prescribing qualification
• Experience of working within General Practice

• Experience with EMIS

• A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
• Independent prescribing qualification
