Advisa Care is a dynamic and fast-growing Home Health and Hospice Agency seeking a Full time Home Health Registered Nurse (RN) to join our dedicated team.

As a Full time Home Health Registered Nurse (RN) at Advisa Care, you will be responsible for providing quality and compassionate care to our patients in the comfort of their own homes. You will work closely with a multi-disciplinary team to develop and implement personalized care plans, provide skilled nursing services, and ensure the highest level of patient satisfaction.

• Conduct initial Start of Care assessments and develop individualized care plans
• Provide skilled nursing services according to physician orders
• Monitor patient progress and update care plans as necessary
• Collaborate with other healthcare professionals to coordinate and manage patient care
• Educate patients and their families on proper self-care techniques
• Ensure compliance with all medical orders and agency policies

• Valid Registered Nurse (RN) license
• Minimum 1 year of home health experience (preferred)
• Excellent clinical assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
• Ability to travel to patient homes within the assigned service area
• Current CPR certification
• Clear criminal background check and drug screen
