With over 18 clinics across Oregon and growing, we’re seeking passionate Nurse Practitioners and Physician Assistants to deliver functional, patient‑centered primary care.

• Provide direct patient care through office visits, telehealth, and occasional same‑day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (Next Gen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team‑oriented environment

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or relevant specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience a plus)
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast‑paced, collaborative environment
• Proficient with electronic health records (Next Gen experience a plus)

Salary Minimum: 125000 Salary Maximum: 160000
