The Community Coordinator connects partners across systems so that families involved in the child welfare system receive aligned, coordinated, wraparound services. You will be a consistent, strong advocate for the developmental needs of infants and toddlers, emphasizing the critical role of early caregiver-child relationships, and you will elevate parent voice throughout the child welfare process.
What You Will Do- Build relationships with community partners to promote coordination, alignment, and integration of services, using effective communication about the Safe Babies Approach.
- Coordinate, plan, and facilitate Active Community Team (ACT) and Site Implementation Team (SIT) meetings, with a focus on reducing disparities and promoting equity.
- Coordinate Family Team Meetings (FTM): invite all necessary participants, provide task-oriented agendas, model a strengths-based approach, and distribute clear written summaries of goals.
- Empower parents to voice their thoughts, ideas, and concerns and to advocate for themselves and their children during FTMs.
- Share information about resources, assessments, evidence-based services, and supports for infants, toddlers, and families, and strengthen referral processes, program alignment, and peer-to-peer learning.
- Carry a caseload of up to 20 families, and attend and convene individual family service meetings for case-managed families.
- Use data trends and de-identified information from the Safe Babies Approach database to inform strategic planning, identify training needs, and work to close gaps in services and knowledge.
- Ensure program intake aligns with Safe Babies Approach standards.
- Participate in local and state initiatives that build community capacity, and in all required training.
- Work with community partners to strengthen families and prevent repeat involvement with Child Welfare.
- Communicate professionally and contribute to a positive, team-oriented culture, and complete other duties as assigned.
- Bachelor's degree in social work, human services, early childhood education, psychology, sociology, public health, or a related field.
- Three years of related work experience.
- Experience facilitating meetings or coordinating services among multiple agencies or partners.
- Experience working with diverse populations, including families affected by trauma, poverty, or involvement with child welfare.
- An equivalent combination of experience and education that demonstrates the required knowledge, skills, and abilities will be considered.
- Master's degree in a related field.
- Five years of work experience in child welfare, early childhood, or a related community-based setting.
- Knowledge of, or training in, infant and toddler development, trauma-informed practice, or the Safe Babies Approach.
- Experience in case management, data tracking, or working in a collaborative, systems-change environment.
- Familiarity with Douglas County community resources and service providers.
- Knowledge of the child welfare system, including dependency court processes and case planning; infant and toddler development and early brain development; the impact of trauma and toxic stress; and family-centered, strengths-based, trauma-informed practice.
- Knowledge of equity, disparities, and cultural responsiveness; collaborative team models and cross-system coordination; and the Safe Babies Approach (or the ability to learn it quickly and apply it with fidelity).
- Skill in facilitating meetings, building relationships with diverse partners, written and verbal communication, active listening, conflict resolution, and consensus building.
- Skill in using data and databases, managing a caseload, organizing and prioritizing multiple tasks, and using standard office software and data entry systems.
- Ability to empower parents, model respect, empathy, and cultural humility, maintain confidentiality, work independently and as part of a team, adapt to changing priorities, and work effectively in high-stress situations involving families in crisis.
- Valid driver's license with a driving record that meets UCAN's acceptable driving guidelines. You will drive a company vehicle to community meetings and partner facilities.
- Must pass a criminal background check prior to hire and meet all screening standards required by the department or program funding sources.
