Job Title: Inpatient Medical Social Worker (LMSW)
Pay Rate: $56/hour (Travelers)
- Day Shift | Monday–Friday
- No on-call requirement
- Optional weekends available if desired and needed
Role Overview:
The Inpatient Medical Social Worker (LMSW) will provide comprehensive social work services to adult and geriatric patients in an acute inpatient setting. The role includes psychosocial assessments, discharge planning, care coordination, and collaboration with interdisciplinary teams to support safe and effective patient transitions.
Key Responsibilities:
- Perform biopsychosocial assessments
- Assist patients and families with discharge planning
- Coordinate care with home health services and durable medical equipment providers
- Facilitate referrals to rehabilitation and long‑term care facilities
- Provide education on and access to community resources (Medicaid, TANF, AFDC, Food Stamps, Medication Assistance, etc.)
- Participate in care conferences and interdisciplinary rounds
- Document patient interactions and care plans accurately in the medical record
Float Requirement:
- Float throughout the patient tower across Med‑Surg units as needed
- Adults and Geriatrics
- Average daily census: 24 patients
Required Qualifications:
- LMSW with active Maryland State License
- BLS Certification
- Minimum 2 years of inpatient medical social work experience (required)
- Experience with discharge planning, rehab and long‑term care referrals
- Knowledge of adult and child protective services, guardianship, power of attorney, and emergency detention
- Strong care coordination and collaborative care experience
- COVID‑19 vaccine not required
- Guaranteed hours
- Modules:
Up to 4 hours billable (additional hours non‑billable) - Orientation provided
- Requests for time off must be submitted at the time of submission (no changes after)
- Maximum 7 days RTO per 13‑week contract (no RTO in first two weeks preferred)
