Pay Range: $90,000-$115,000 per year for full-time based on experience.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps develop treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs. Assist physicians in developing the therapy plan of care.
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to accepted standards of practice.
• Instruct and inform healthcare team members, patients, and families about the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the relevant state
• Current CPR certification
• Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle
