Report To:
Director of Nursing

Under the direction of the Director of Nursing, the CNA shall perform functions that enhance services related to the Medical Care and Activity Programs provided daily to each Member of the Adult Day Center.

• Engage with Members through constant interaction, including observation throughout the day, assisting with meal preparation, serving, and feeding if needed.
• Encourage and engage Members in activities, exercise, and music/movement classes.
• Provide support to ensure Members' safety, health, and well-being.
• Monitor vital signs and document as needed.
• Assist Members with Activities of Daily Living (ADLs) such as toileting, clothing changes, and feeding.
• Help prepare, implement, and evaluate Member activities.
• Assist Members with crafts, exercise, and music/mobility classes.
• Accompany Members to functions outside the Center if needed.
• Serve meals and snacks.
• Assist with outdoor activities and trips to stores.

• Fluent in English and Spanish.
• 1-3 years of experience.
• Certified CNA or CHHA.
• Strong interpersonal skills, self-motivated, enthusiastic, and well-organized.
• Ability to monitor multiple projects and assist with a multi-level functioning program.
