Welbe Health PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Person al Care Assistant is a vital part of our team, providing aid to our participants and contributing to the care planning process as a member of our interdisciplinary team (IDT).

Reporting to the Day Center Manager, the Personal Care Assistant ensures safe delivery of personal care services to participants at the Welbe Health day center.

• Collaborate with different departments within our day centers, building strong connections with both participants and team members.
• Care for seniors in a lively day center environment where participants receive full-service care ranging from medical appointments to rehabilitation services, games, and activities.

• Provide hands‑on personal care assistance with activities of daily living (ADLs) to Welbe Health participants, including escorting, toileting, bathing, dressing, grooming, laundry, and physical lifting when transferring participants.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed, assisting participants with boarding and exiting Welbe Health transportation vehicles.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service, ensuring participants receive the correct meals, and feed when needed.
• Maintain personal care supplies such as incontinence supplies and grooming items.

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in California preferred.
• Minimum of one (1) year of experience providing personal care to seniors.
• CA Food Handlers Certification within 30 days of hire.
• Proficiency in Mandarin and English or Cantonese and English.

Salary/Wage base range for this role is $21.00 – $25.22 hourly plus bonus and equity. Actual pay will be adjusted based on experience and other qualifications.
