Job Overview
Welbe Health PACE invites a licensed RN to serve as Case Manager, RN. The role guides participant care within a value‑based model, collaborating across an interdisciplinary team (IDT) to enhance quality and compassion for seniors.
Day‑to‑Day Responsibilities- Conduct nursing assessments to determine participant needs, preferences, and goals.
- Participate in IDT meetings to develop and document participant care plans.
- Deliver and document interventions, including medication administration, safety monitoring, physician order compliance.
- Coordinate care with the Welbe Health PACE Physician and IDT members, revising plans with participants and families.
- Educate participants, families, and caregivers on disease processes, self‑care techniques, and prevention strategies.
- BSN from an accredited nursing program (preferred).
- Current California RN License.
- Current BLS/First Aid certification.
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population (preferred).
- Medical, Dental, Vision insurance.
- 17 days of personal time off, 12 observed holidays, 6 sick days.

Hourly base range: $52.52 – $69.32 + bonus + equity. Total rewards include 401(k) match, comprehensive medical coverage, and other benefits.
