• Excellent Pay: $200 per Start of Care visit + mileage
• Focused Role: Specialize in new patient admissions and care plan initiation
• Community‑Based: Get out of the office and into the homes of clients across the metro
• Professional Independence: Use your clinical judgment to help patients begin their care journey

• Perform weekend Start of Care (SOC) admissions with complete OASIS documentation
• Develop and initiate care plans in collaboration with the interdisciplinary team
• Evaluate patient needs and coordinate necessary services
• Educate patients and caregivers on disease management and home care best practices

• Valid Minnesota RN license and CPR certification
• Proven experience in home health and OASIS documentation
• Reliable transportation, driver's license, and active insurance
• Strong communication and clinical assessment skills
