Home Health Care, Inc. is seeking a dedicated LPN Visit Nurse for full-time field work in the Twin Cities metro area. You will perform skilled nursing visits in patients' homes, manage medications, educate families, and document care provided during each visit.
You'll travel within a ~35-mile radius of your home, collaborating with RN Case Managers, physicians, and therapists to ensure continuity of care and patient safety.
