Care Manager

Job Summary:

The Care Manager is responsible for overseeing a caseload of incapacitated adults for whom the agency serves as the legal guardian. This role requires compassion, organization, and the ability to manage a dynamic workload while advocating for the well-being of clients.

Key Responsibilities:

• Manage an assigned caseload of incapacitated adults under agency guardianship.
• Conduct in-person visits with each client at least once per month. Care Manager will need to use their own car while working out in the field so reliable transportation is a must.
• Participate in care-related meetings and interdisciplinary planning sessions.
• Be available to respond to phone calls and text messages regarding clients, with some flexibility outside of standard business hours.
• Complete and maintain required documentation, including:
• Case Notes
• Monthly Time Sheets and Mileage Sheets
• Initial Assessments
• Home Environment Assessments
• Annual Care Plans
• Annual Reports for court submission

Qualifications:

• Experience or familiarity with working with adults who have intellectual and/or developmental disabilities is highly preferred.
• Comfort with making medical decisions for clients.
• Strong communication, time management, and documentation skills.
• Ability to work independently while maintaining compliance with legal and agency requirements.
